Annual Certification - Action Required

This article provides guidance on what to do when your annual certification requires further action. You will receive an error message when submitting your annual certification report if:

• You have unsubmitted prior approval forms (Category I, Student Involvement, etc.)
• No effort has been reported on an activity
• Academic Year Faculty: Did not complete the Summer Hours Info section in UC OATS

Unsubmitted Prior Approval Forms

An error message will appear on the "My Annual Certifications" tab when you have a form that needs to be submitted.  All prior approval forms must be submitted before you can complete your annual certification. 

To fix the error:

1. Click on the review link after the error message or select the My Activities and Forms tab.
2. Locate the activity in your workspace that has the resubmitted form. Complete and submit the Category I form or delete the activity. Once this is complete you can submit your Annual Certification.

No Effort Reported

If there are activities in your workspace with no hours reported, an error message will appear at the bottom of the Annual Certification report. While you are not required to add hours, it’s recommended to add an activity note if you did not engage in the activity. This note will help reviewers and approvers understand why no effort was reported for that activity.

Academic Year Faculty: Summer Info

Academic Year Faculty: Academic year faculty must fill out Summer Info for the fiscal year they are certifying before completing annual certification.
